The Stepped Care Model for Mental Health Support

More Accessibility – More Precision – More Help

The Stepped Care Model offers an innovative approach to providing personalized support, grounded in the principles of flexibility, continuity of care, and optimal use of resources.

Instead of a one-size-fits-all solution, the model enables tailored mental health support at varying levels of expertise and complexity—ranging from peer support, information, guidance, and resilience-building tools provided by Community Mentors, to moderate-level interventions and mental well-being promotion by Psychological Wellbeing Practioners (WPs), and up to professional mental health care delivered by qualified experts.

"Community Mentors" offering support, information, guidance, and assistance, is a program of both human and professional innovation. It expands the scope of psycho-social support within the community by integrating mentors into the welfare, education, and mental health systems.

The program is designed to reinforce existing services, enhance continuity of care for those in need, and ease the burden on the public system.

Through targeted training and integration of mentors in the field, an additional layer of support is created—offering initial guidance for individuals seeking help, assisting in identifying at-risk populations, and making psycho-social support more accessible.

"When one person shows up for another, a meaningful connection is formed—
one that strengthens resilience and supports
mental well-being.

The 'Community Mentors' is the first step program in a broader model
that offers a continuum of care—both
personal and professional."
